The Community Outreach Specialist from PCHC will provide enrollment support for people seeking primary medical, behavioral health, dental, and pregnancy care services a first-come, first-served basis.
Pueblo Community Health Center (PCHC) is excited to offer enrollment support for all clients who utilize the library. The Community Outreach Specialist from PCHC will be available to register people who need primary medical, behavioral health, dental, and/or pregnancy care services. An intake appointment will be set up at the patient’s preferred clinic location once enrollment is completed. PCHC has sites throughout Pueblo (including Avondale, Pueblo Community College, and its newest clinic at East 8th and Lacrosse Streets). If an individual doesn't have insurance, a financial assistance appointment will also be scheduled to determine programs for which the patient is eligible (including the PCHC sliding fee discount program or Health First Colorado/Medicaid). Pueblo Community Health Center has been the same since 1983—to provide primary care services to those in need—regardless of ability to pay.
